EVLT stands for Endovenous Laser Treatment. It is a minimally invasive treatment, which closes the affected vein by heating the inside walls of the affected veins with a laser. This treatment is by far one of the safest compared to traditional surgeries such as sclerotherapy. The patients are almost immediately relieved of their symptoms.

What are the pre-treatment preparations for EVLT?

Pre-treatment procedures are conducted so as to smoothly carry out the treatment without any complications. It involves the following:

  • The doctor may advise the patient to stop taking any medication (especially blood thinners) before the surgery.
  • The patient should avoid forcing their weight on the affected leg before the surgery.
  • No food or drink should be consumed by the patient for a minimum 8 hours before the surgery.
  • Before starting treatment, the physician will scan the problematic area of the skin using a hand-held ultrasound device. This is done to identify the parts of the vein that have varices, so as to get a clear idea of where the treatment should be concentrated.

What happens during the EVLT procedure?

The EVLT procedure is minimally invasive. It is a very short procedure which does not require hospital stay. A simple overview of the surgery is as follows:

  • The patient is given a local anaesthetic, so as to numb the portion to be treated.  
  • The skin is sterilized and the needed instruments are guideed into the affected veins. It comprises of 3 stages- 1.Guide wire is positioned inside the target vein. The main job of the guide wire is to guide the catheter (flexible tube) to the treatment site.   2. Once the guide wire is in place a sheath is placed around and runs along the length of the wire. 3. After the sheath completely covers the guide wire, the wire is then pulled out and the laser is then inserted.

Once the instruments are inserted

  • The laser is activated and gently pulled back along the length of the varicose vein.
  • Due to the heat emitted by the laser, the walls of the vein collapse behind the tip of the laser as it is pulled back thus, closing the vein.   

What kind of care is required post treatment?=

Once the vein is treated, the sheath and the laser are removed. The symptoms are relieved almost immediately. However, to make this relief lasting and permanent, doctors advise the patients on how to manage themselves after EVLT. It includes:  

  • Wearing compression stockings for upto 2 weeks after the surgery. The motive behind this is to create a pressure which will assist the leg veins to push the blood up, thus preventing a venous disorder to occur again.
  • To reduce the pain, painkillers will be prescribed.
  • There will be swelling seen on the treated site after the surgery. It is recommended to keep an ice pack on the site for a day or two after the surgery to reduce the swelling.
